The pathogenesis of obesity is complex, with contributions from genetic, environmental, and psychosocial factors. In a study of 1850 twin pairs (one-third monozygotic), the heritability of body mass index at four years of age was greater among children living in home environments at high obesogenic risk, based upon food, activity, and media exposures during early childhood, compared with those living in healthier environments (86 versus 39 percent, respectively) . These findings suggest that a healthy home environment during early childhood may attenuate the effect of a genetic predisposition to obesity.
Cannabis is the most widely used illicit drug among adolescents, and it has been reported to be associated with some negative consequences. In a metaanalysis of 11 prospective longitudinal studies (more than 23,000 adolescents) examining the association between adolescent cannabis use and the risk of developing a mood disorder or suicidality as a young adult, there was a significant association between cannabis use and increased risks of depression, suicidal ideation, and suicide attempt (but not anxiety) . Additional research evaluating the potential adverse effects of cannabis use in adolescents is needed.
In addition to the known hazards of maternal smoking on pregnancy outcome, smoking is a modifiable risk factor for sudden unexpected infant death (SUID, infant death <1 year of age). In a United States birth cohort study, even smoking just one cigarette daily throughout pregnancy doubled the risk for SUID . Moreover, women who reduced or quit smoking during pregnancy cut the SUID risk by 12 and 20 percent, respectively. This study provides additional support for our practice of asking all women about smoking habits and offering smoking cessation treatments regardless of pregnancy status.
